A - Patient requesting: Investigation Reports

If the patient wants a copy of their test report (such as Blood test, X-Ray, Ultrasound, MRI, CT etc) ordered by one of the physicians at our medical facility, they can can do so by:

Option - 1: See Family Doctor or equivalent

If the patient is unable to visit our clinic, the patient can visit their family doctor who can obtain the investigation report directly by contacting respective department :

  • Blood test, Urine, Swab, Stool, Sputum, PAP etc. Dynacare - (905) 790-3030
  • Ultrasound, X-Ray, Bone Density, Upper GI series (barium swallow). Queensway X-Ray & Ultrasound - (905) 897-6970
  • For specialized tests done outside of our facility (such as CT, MRI, Mammogram etc), contact the testing centre directly.
  • Please note that each facility may have their own documentation requirement to release medical information.
Option - 2: Release of medical records form

Patient should fill the Release of medical records form to have their medical information mailed to an address of their choosing. The fee associated for this request and payment method can be found on the Release of medical records form. Once the form is filled, it can be submitted in 1 of 2 ways:

  1. MAIL-IN: Patient can mail-in the completed form, where the signature of the patient has been notarized by a notary or equivalent, along with a copy of 2 pieces of valid photo ID that belongs to the patient.

    B - Patient requesting: Record of patient visit (Patient Chart)

    Record of patient visit (Patient chart) contains medical information documented by physician during their encounter with the patient. It also contains Physician name, physician signature and date of the visit.

    For patients requesting a copy of the consultation report, they may do so via the following methods:

    Patient should fill the Release of medical records formto have their medical information mailed to an address of their choosing. The fee associated for this request and payment method can be found on the Release of medical records form. Once the form is filled, it can be submitted in 1 of 2 ways:

    1. IN-PERSON BY THE PATIENT: Completed form can be submitted in-person at our office between the hours of 9AM-3PM Monday to Friday (excl. Holidays) and will require 2-pieces of valid photo ID that belongs to the patient in question.

The fee associated for this request and payment method can be found on the Release of medical records form. Once the form is filled, it can be submitted in 1 of 2 ways:

    1. IN-PERSON BY THE PATIENT: Completed form can be submitted in-person at our office between the hours of 9AM-3PM Monday to Friday (excl. Holidays) and will require 2-pieces of valid photo ID that belongs to the patient in question.

    J. Referring physician (Physician who initially referred the patient to consult with a specialist at our facility) requesting: Record of patient visit (Patient Chart)

    The parents of the patient, should fill the Release of medical records to parent (patient under the age of 18) form to have the patient medical information mailed to an address of their choosing. The completed form must be submitted via the following method.

    IN-PERSON BY ONE OR BOTH PARENT(S): Completed form can be submitted in-person at our office between the hours of 9AM-3PM Monday to Friday (excl. Holidays) and will require 2-pieces of valid ID per parent and 2-pieces of ID that belongs to the patient in question.

    Note: If both parents are not present at the time of form submission, at least one of the IDs that belongs to the parent that is absent must contain parent signature. We require this to verify parent signature on the completed form.




    Processing Time:

    IN-PERSON BY ONE OR BOTH PARENT(S): Completed form can be submitted in-person at our office between the hours of 9AM-3PM Monday to Friday (excl. Holidays) and will require 2-pieces of valid ID per parent and 2-pieces of ID that belongs to the patient in question.

    Note: If both parents are not present at the time of form submission, at least one of the IDs that belongs to the parent that is absent must contain parent signature. We require this to verify parent signature on the completed form.

    IN-PERSON BY ONE OR BOTH PARENT(S): Completed form can be submitted in-person at our office between the hours of 9AM-3PM Monday to Friday (excl. Holidays) and will require 2-pieces of valid ID per parent and 2-pieces of ID that belongs to the patient in question.

    Note: If both parents are not present at the time of form submission, at least one of the IDs that belongs to the parent that is absent must contain parent signature. We require this to verify parent signature on the completed form.




    Processing Time:

    2-4 weeks for medical records that are less than 3 years old.

    4-10 weeks or longer for medical records older than 3 years.

    It is the parent's responsibility to make sure they allow enough time for processing. These requests cannot be rushed and medical records will only be sent via mail.

    O. Parent requesting medical records for patient who is under the age of 18 requesting: Investigation report, record of visit (patient chart), consultation report or all of the above

    The parents of the patient, should fill the Release of medical records to parent (patient under the age of 18) form to have the patient medical information mailed to an address of their choosing. The completed form must be submitted via the following method.

    Special Cases (Custody parent, legal guardian, court order release etc.)

    Documentation requirements for these cases are determined on a case-by-case basis. Please visit our clinic Monday - Friday 9am - 3pm (excl. holidays) to meet with the Medical record Manager to determine what kind of documents are required.
